Updated Preseason Information for Soccer and Field Hockey
All try-out sessions for soccer and field hockey on August 19th, will now take place on Grellier Field with its artificial surface.
Please see the updated schedule below for new check in times and try-out sessions for field hockey and soccer.
Check-in will be in the Athletic Center.

8:30 a.m. Check-in begins for women’s soccer—Athletic Center
9:00 a.m. Try-out session for women’s soccer—Grellier Field
10:30 a.m. Check-in begins for field hockey--Athletic Center
11:30 a.m. Try-out session for field hockey Grellier
11:30-2:30 p.m. Lunch in Valentine Dining Hall—All family members are invited to lunch.
12:30 p.m. Check-in begins for men’s soccer—Athletic Center
2:00 p.m. Try-out session for men’s soccer--Grellier
On behalf of the Athletic Training Staff, welcome to Lasell College and congratulations on becoming a member of the Lasell Community. We strive to provide the highest quality Athletic Training services for our student-athletes. The first step toward doing that is to understand your medical history. It is also important that we have each student-athlete’s medical insurance information on record in case of injury, illness or emergency that requires medical services beyond those we provide on-campus.

All NCAA athletes are required to have a general medical physical by their primary care physician. The NCAA requires that the physical must have been done within the previous 6 months! There are NO exceptions to this rule. Your physical form MUST be dated after March 1, 2012 in order to participate.
THESE FORMS ARE DIFFERENT FROM THE HEALTH FORMS THAT YOU WILL RECEIVE FROM THE COLLEGE AND SHOULD BE SUBMITTED SEPERATELY!
Please use the following check list to make sure that your paperwork is complete:
* Physical Exam and Immunization Form
* Athletic Health History Questionnaire
* Emergency Medical Information/Demographic Form
* Authorization for Disclosure of Protected Health Information
* Medical Consent, Sports Safety and Medical Statements
* Insurance Statement
* Front and back copy of Insurance Card
* All documents must be completed, dated and signed by the athlete (and parent/guardian if student-athlete is under 18)
* NCAA ADHD Requirements – only complete if you are currently on any medication for ADHD
* NCAA Sickle Cell Reporting – only complete if you are prone to this disease.
All paperwork must be completely filled out and returned to the Athletic Training Office no later than August 1, 2012. You will NOT be able to participate in athletics until all of your paperwork is properly completed and verified. Submitting paperwork late may affect your eligibility to try out or play for your team.
