Medical Eligibility Forms

All forms for new and returning student-athletes are due August 1st, 2014.  All forms can be accessed through the links below.

First Year and Transfer Athletes

In order to participate in athletics, all first-year and transfer athletes must have:

• Physical exam dated within six months of initial participation

                     Physical Exam and Immunization Form 

• Front and Back Copy of your insurance card
• Completion of medical eligibility packet 

• You must also complete EITHER the Sickle Cell Waiver Form or the Sickle Cell Disclosure Form

If applicable please complete the following documentation regarding ADD and ADHD

• NCAA ADHD Medical Exception Form for Physicians
• NCAA ADHD Policy/Information/Medical Exception Form

Returning Athletes (All forms are to be returned to the Athletic Training Office.)

All returning student athletes must complete:

• Returning Athlete Health History Form
• Demographic Information
• Returning Athlete Medical consent and Insurance Statement

You must also complete EITHER the Sickle Cell Waiver Form or the Sickle Cell Disclosure Form

• Sickle Cell Disclosure Form
• Sickle Cell Waiver Form

**Also any additional information regarding changes in insurance coverage.**
